You will find the MSU Drivers Certification form using the link below. Email your completed form to CSUS.facilities@msu.edu.

If your drivers license has been issued in a state other than Michigan, please submit your 36-month driving record from your issuing state along with your completed drivers certification form to CSUS.facilities@msu.edu.
